How To Apply

To apply for the National School Lunch Program and School Breakfast Program there are four steps that must be followed:

Step 1: Identify type of school below for the corresponding list of items needed:

Charter Schools must have the following to apply:

  1. SAM/DUNS Registration - Click here for information and website
  2. Online Substitute Form W9 – Vendor Registration - Click here for information and website
  3. My Florida Market Place - Click here for information and website
  4. Copy of signed Charter School agreement between the Charter school and the school district
  5. School must be registered with the Florida Department of Education
  6. Department of Health Food Operating Permit – Please contact your local health department

Private Non-Profit Schools must have the following to apply:

  1. SAM/DUNS Registration - Click here for information and website
  2. Online Substitute Form W9 – Vendor Registration - Click here for information and website
  3. My Florida Market Place - Click here for information and website
  4. IRS Letter of Tax Exemption – 501(c)(3)  http://www.irs.gov/Charities-&-Non-Profits/Application-for-Recognition-of-Exemption
  5. School must be registered with the Florida Department of Education
  6. Department of Health Food Operating Permit – Please contact your local health department

Residential Child Care Institutions must have the following to apply:

  1. SAM/DUNS Registration - Click here for information and website
  2. Online Substitute Form W9 – Vendor Registration - Click here for information and website
  3. My Florida Market Place - Click here for information and website
  4. IRS Letter of Tax Exemption – 501(c)(3)  http://www.irs.gov/Charities-&-Non-Profits/Application-for-Recognition-of-Exemption
  5. Department of Health Food Operating Permit – Please contact your local health department

Step 2: Complete Prospective Sponsor information form and email

  1. Complete this form – Click here for Prospective Sponsor form
  2. Once completed email to NSLPprospectivesponsor@freshfromflorida.com

Step 3: Prospective Sponsors of the National School Lunch Program and School Breakfast Program must register and attend a School Lunch training, as well as a NSLP Web training.

Step 4:  After attending training Prospective Sponsors must complete the required forms and submit an On-line Application for the National School Lunch Program.

  1. Submit the required documents for review – located at https://cnp.freshfromflorida.com/FloridaPrd/FormDownload/FormDownloadMngr.asp?Mode=NSFD&ProgramGrpId=NSLP
  2. Once your required documents have been submitted and approved, an onsite pre-operational visit will be set up with your Program Development Specialist
  3. Complete the Online Child Nutrition Program module
  4. Once your online application has been reviewed and approved, you will receive an Approval Confirmation from your Program Development Specialist.
  5. You may then begin claiming reimbursable meals served to the students.
